All NIHB plan members (clients) ages 2 years and older managing their type 1 or type 2 diabetes with insulin are eligible for Dexcom G6 or Dexcom G7 coverage.

All eligible clients must have prior approval before they can get their Dexcom rtCGM supplies from their pharmacy.
