Patients living with type 1 diabetes aged 2 years or older who meet one or more of the

following criteria: 

• Failure to achieve an appropriate glycated hemoglobin (HbA1c) value despite taking prescribed medications and testing blood sugar regularly; or
• Frequent episodes of hypoglycemia in the past year despite following a treatment plan; or
• Inability to recognize or report symptoms of hypoglycemia.
