Doctors, nurse practitioners (NP) and eligible prescribing pharmacists may initiate the process by giving the client a Dexcom G6 or Dexcom G7 prescription they can take to their pharmacy. 

The pharmacist will contact the NIHB Drug Exception Center (DEC) at 1-800-580-0950 to check and confirm the insulin claims history. If approved, the DEC will fax an approval confirmation to the pharmacy so that direct billing may proceed. 

A Limited Use (LU) form may be required by the DEC if the claims history does not show that NIHB has paid insulin claims for the client.
